Know Your Numbers Biometric Screening & Wellness Program


Keuka College, MVP and Wellness Programs with Value (WPV) have partnered together to help you "Know Your Numbers" through our biometrics screening and wellness program.  Flu shots will also be available. You do not have to sign up for a screening to receive a flu shot. Walk in times for flu shots will be 10:00am - 2:00pm.

The first step in taking charge of your health is to Know Your Numbers, which indicates your overall health. Keuka College encourages you to take this important step by offering free biometric screenings to our employees.

These comprehensive, confidential screenings are conducted by a registered nurse. All that is involved is a quick finger stick and you will receive instant results, and a thorough explanation of your:

  • Total Cholesterol

  • Hemoglobin A1C (blood glucose)

  • Blood Pressure, Heart Rate, Oxygen Saturation

  • Weight, Body Mass Index and percentage of Body Fat

November 15th 9:00pm - 3:00pm Ball Hall, 1st floor, suite 131

To participate in an on-site Biometric Screening you will need to:

  • Bring your MVP Insurance Card. If you do not have MVP insurance please inform the nurse performing your screening;

  • try to fast 2 hours prior to your scheduled appointment time, however fasting is not required;

  • click on one of the registration links below to register for your 15 minute screening appointment.

If you unable to attend an onsite screening you may submit the following form, completed by your Primary Care Physician: Biometric Screening PCP Results to participate. The information provided must be from a visit between October 1, 2018 through November 15th, 2019. Forms must by submitted by November 15th to be included in the program. Information for submitting the forms to WPV are on the form itself.


To receive a flu immunization you will need to bring your insurance card. The following insurances are accepted: Aetna, BC/BS, CDPHP, Cigna, Excellus, Independent Health, LBS, Medicare, MVP, United HC and Univera. If you do not have an accepted insurance, you may pay $29 cash or check at the time of your immunization.

If you have any questions, or you are unable to attend the clinic you signed up for, please call 585-568-8340 to get help with on-line sign-up or to cancel your appointment. Help is available between 9 AM through 5 PM, Monday through Friday.
