Know Your Numbers Biometric Screening & Wellness Program

As part of our ongoing commitment to ensuring the health & well-being of our employees, Lewis County is excited to partner with a local premier wellness company, WPV, to offer all benefit-eligible employees an opportunity to learn more about your current state of health.

Knowing your numbers is one of the best ways to evaluate your health by identifying your risk and providing motivation for managing your health. Even if you feel great and look healthy, you may not know what's going on inside. Many diseases are "silent" and affect your health long before you feel anything wrong.

These comprehensive, confidential screenings are conducted by a registered nurse. All that is involved is a quick finger stick and you will receive instant results, and a thorough explanation of your:

  • Total Cholesterol, HDL, TC/HDL Ratio

  • Hemoglobin A1C (blood glucose)

  • Blood Pressure, Heart Rate, Oxygen Saturation

  • Weight, Body Mass Index and percentage of Body Fat

We will also be performing a saliva Cotinine Test for those who have signed that they do not use tobacco products on the Tobacco Attestation Form.

Before coming to your screening we will ask that you fill out a short Health Risk Assessment to review with the nurse.

To participate in an on-site Biometric Screening you will need to:

  • Bring your Excellus Insurance Card

  • Try to fast 2 hours prior to your scheduled appointment time, however fasting in not required

  • Click on one of the registration links above to register for your 15 minute appointment

  • Bring your completed Health Risk Assessment form

If you have any questions, or you are unable to attend the clinic you signed up for, please call 585-568-8340 to get help with on-line sign-up or to cancel your appointment. Help is available between 9 AM through 5 PM, Monday through Friday.
